4g connection suddenly stopped on my Samsung note 4

A couple of days back I lost connection to 4g on my note 4. I've done the following :

1. Called Maxis (local service provider) and increased my data plan thinking I've run out.


2. Checked with Maxis to see if anything wrong with my Internet data plan. They said everything ok.


3. Made sure I'm not on data roaming.


4. Powered off and rebooted countless times.
